Case Manager for the elderly with multimorbidity - Importance for continuity of care
Overall aim of the thesis
The overall purpose of the study is to explore the Case Manager function from multiple perspectives, with a particular focus on the Case Managers experience, the elderly with multimorbidity and their informal caregivers experiences of the function and its importance for the continuity of care.
